WHAT TO EXPECT

Below is helpful information pertaining to commonly asked questions.  Please review this information and feel free to contact us with any additional questions and/or concerns.

YOUR FIRST APPOINTMENT

Your first telehealth appointment could last up to 60 minutes depending on the reason for the visit.

During this initial visit, we will take the time to discuss your needs/preferences and develop a treatment plan geared towards reaching these goals.


Having online forms submitted 2 days prior to your appointment allows the visit to be about you and not about filling out forms. We will review all of your information prior to your appointment to ensure a positive and relaxed experience.

INFORMATION NEEDED

Specific forms necessary for your first visit will be reviewed during the FREE 15-minute initial phone consult and are dependent on the services being requested.

Forms must be filled out and sent to OPTIMUM Psychiatric Health via secure Patient Portal, email, or fax, at least 2 business days prior to your scheduled appointment time. If you prefer to use the Patient Portal we will email you a link to the appropriate forms for online submission. If you prefer to fax your forms, our secure fax line is (949) 561-4522.

If you are being seen for a medication management (prescriber) visit, we request a copy of your most recent physical and medication list from your current Primary Care Physician (within the past 1 year), along with the last visit note and psychiatric medication list from your prior/current mental health provider.

​

We may also request labs/bloodwork and/or UDS, yet will provide instructions on how to complete these.
